If you harbor concerns that your hypnotherapist may have engaged in unethical conduct while providing therapy services, you retain the right to file a complaint.
Step 1 - Please refer to CHA’s Code of Conduct to ensure the therapist has breached the code.

Step 2 - Search our directory to ensure they are part of this registry.

Step 3 - Open the form and file your complaint with as much information as possible. All information is kept secure and confidential.

Complaints against healthcare practitioners not affiliated with the CHA should be directed to the Health Complaints entity in your respective state or territory.
